Thought goes, that to make the pain easier, Dean pushes Sam away, so's that he won't miss him as much when Dean has to 'go away'. Eventually Dean leaves Sam by a roadside, leaving Sam in a pit of guilt and sadness. But, Dean comes home to Sam for one final goodbye.
In other news: the idea of home for the boys, is being with each other, after all, home is where the heart is!
