I use a program called FeedBack to make my own charts and GHX (Guitar Hero Explorer) to put them into the game.
I do not have a modded PS2. I use Swap Magic to play burned games. It works great. You can get a copy online anywhere it's sold for about $40 with shipping. I got mine on EBay. It's worth every penny. Puts a lot of replay value into Guitar Hero.
You can also get full custom games that other people have already made from bit torrent sites. I got a couple games from this source also.
